The following forms are available as .pdf files for you to download and print. You will need Adobe Reader to open these files. Adobe Reader is free and can be downloaded at the Adobe website.

Please note the following:
1. The forms available on this page are the same ones you would receive if you requested them through the camp office. Please feel free to contact us if you have additional questions.
2. Registration forms can be filled out on your computer and emailed, printed and mailed, or printed and faxed to camp.
3. Online registration for summer camp is possible through the Registration page; however, because of the need to have hard copies of parental signatures, printing and mailing .pdf files from this page is preferred.

Following registration, our office staff will send you a confirmation with the following documents:

• Confirmation of Registration and Payment
• Medical Form and Medical History Form (required for all campers)
• Medication Administration Release Form (required for any campers bringing medications to camp)
• Camper Personal History Form (recommended for all campers)
• Coaching Period Request Form (Day Camp only)
